Vital Records

Wisconsin birth certificates and marriage certificates are available statewide. If you were born or married in the State of Wisconsin you can obtain your birth certificate and marriage certificate in ANY county.  We also provide statewide issuance for death certificates (9/1/2013-present) and divorce certificates (1/1/2016–present).  We also issue military discharges.

 

Vital records certificates may be purchased in person, through the mail, or via VitalChek (VitalChek link on website).  Applications for copies of vital records must be accompanied by a copy of the applicant’s valid photo ID.

 

The cost for vital records is $20.00 for the first copy and $3.00 for each additional copy per time of each record.
